Application

We are glad you are interested in joining the SBHP registry. This means your basic information will be listed in a registry with the training level you have completed, searchable by anyone looking for services or referring someone for services. You are welcome to apply to the registry at any time but your information is not viewable until you have completed Tier One Training and been accepted to the registry. You must apply and be accepted before attending Tier Three.


The application to the registry includes the following licensed professions: Licensure, in good-standing, as a Clinical Social Worker, Psychologist, Mental Health Counselor, Marriage and Family Therapist, Clinical Nurse Specialist, Psychiatric Nurse Practitioner or Psychiatrist is acceptable.

STEP ONE:

Complete the application. Apply here.

STEP TWO:

After submitting your application, you will receive an email asking you to email your information to admin@starproviders.org or fax it to (517) 432-2953 attention: SBHP. You will need to send proof of licensure and proof of insurance. Once these documents are received they will be reviewed and licensure will be verified.

STEP THREE:

Once the review process is complete, you will receive an email to let you know that you have been accepted into the Registry, or notified of any reasons for a decision to not allow entrance into the registry. The link to the website where your information is available will be emailed to you for verification of the data entered. If information needs to be corrected, email admin@starproviders.org or call (989) 448-1125.
You can apply to the registry at any time before attending tier three training.

STEP FOUR:

Once you are listed in the registry your level of training will be listed, along with your location, specialty, credentialing, areas of focus, populations treated and more. This will allow consumers to easily locate you as a provider who can better meet their needs, and you will begin to enjoy the benefits of membership in Star Behavioral Health Providers. You will be contacted annually to verify that your information remains accurate and up-to-date.

Opt Out Of The Registry:

While this experience should be beneficial for you as the provider, and those whom you serve, you are able to opt out of the provider registry at any time. Please contact admin@starproviders.org or call (989) 448-1125 to request removal from the registry. If you remove yourself from the registry, you will not be able to participate in any additional tier three training sessions.

Inclusion in the Star Behavioral Health Providers does not imply a contractual agreement or specific endorsement of any kind. Participation is completely voluntary on your part and inclusion is at the sole discretion of the Department of Human Development and Family Studies at Michigan State University. Inclusion in SBHP does not imply a promise of referrals, payment, training, or access to any other tangible recourse or benefit. Inclusion means you will have your name added to a list of providers who will be represented by the Department of Human Development and Family Studies at Michigan State University as having met minimal qualifications for working with military service members.
