To be listed on the SBHP registry, you will need to meet certain licensure criteria and successfully complete the application process.

Each state's SBHP team identifies the behavioral health professions which can be included in the registry for that particular state. While each state has some different titles and/or licensing guidelines, in general, licensure, in good-standing, as a Clinical Social Worker, Psychologist, Mental Health Counselor, Marriage and Family Therapist, Clinical Nurse Specialist, Psychiatric Nurse Practitioner, Clinical Addiction Counselor or Psychiatrist is acceptable. To see what professions are included in your state's Registry, please view the Application Page under the "Apply to the Registry" tab.

Application to the registry can be found here. You can apply anytime before attending Tier Three Training. Tier Three is intended for licensed behavioral health providers, at or above the master's level, who want to provide care to service members and their families and accepting new clients into their practice. You will need the following Information in order to complete the application:
  • National Provider Identifier (NPI), if applicable
  • Education Graduation information
    • Name of University
    • Degree type obtained
    • Graduation year
  • Licensing information: Type, issuing body, state issued from, original license date, and expiration date (copy needs to be submitted)
  • Completion date of your supervisory experience
  • Proof of professional liability insurance (copy needs to be submitted)
  • Areas of expertise: Demographics, populations, disorders, etc.

You should receive verification that this information was received within two weeks of sending it. Please contact admin@starproviders.org if you have not received an email within that timeframe.
